Any child with a life-threatening or life-limiting illness requiring subspecialty pediatric care and all of the following may benefit from the services offered by the Hummingbird Program.
- The condition causes significant pain or emotional, social or spiritual distress.
- The condition causes fragmentation of care and communication among healthcare providers.
- The condition disrupts the ability to perform age-appropriate life activities.
- The condition results in frequent emergency department visits, hospitalizations or prolonged lengths of hospital stay.
- The condition requires assistance with complex decision-making and determination of goals of care.
