Care Coordination |  The Penn State Milton S. Hershey Medical Center is committed to an interdisciplinary approach to managing patient care. The Care Coordination Department responds to the medical management and psychosocial needs of all patients served by the Medical Center to achieve the highest quality and most customer-centered outcomes throughout the continuum of health care.
Care Coordination staff are assigned to every unit of the hospital as well as certain specialty services. Care Coordination staff consist of the Care Coordination Director, the Care Coordination Manger, Social Workers, Care Coordinator Nurses, Advance Practice Nurses, and Administrative Staff.
What We Do:
- Provide care in an efficient, effective and quality manner for each patient at any given time in illness and/or wellness
- Manage care proactively, matching appropriate internal and external resources to the patient’s medical and psycho-social needs
- Empower patients and their families to become active, informed partners in the care and health maintenance
- Contribute to improving care delivery at a World-Class level
- Serve as a resource for patients and staff for addressing needs of victims of violence across the lifespan
- Evaluate services provided based on ethical practice, patient needs and rights, insurers’ expectations, and best practice standards
